Instructions to Obtain a Death Certificate

Death Certificates are issued in the County Clerk’s office, located at 150 N. Seguin Ave., New Braunfels, Tx 78130.

                             The Vitals Statistics Department hours are:

8:00 a.m. – 4:30 p.m.

Monday - Friday

 

 

A Certified Copy of a Death Certificate is $21.00 for the first and $4.00 for each additional and will be charged to cover the cost of searching the record.  If a record is located, the certified copy is issued.

 

Death records are CLOSED for 25 years from the date of death.

A qualified applicant may make application to receive a certified copy.

 

The Texas Board of Health has the authority to control accessibility to death certificates and has adopted rules defining who may be qualified to obtain certified copies.  [HSC§191.051]

 

The Texas Administrative Code [25§ 181.1] defines a qualified applicant as follows:

·        a person of his or her immediate family either by blood or marriage,

Father

Brother

Sister

Daughter

Son

including step parents and step siblings,

·        his or her legal representative or agent.

 

Each applicant must present a current valid form of photo identification (government issued) along with the required application.  [25TAC§181.1]

 

Persons not meeting the criteria as a qualified applicant may make application and receive a “Verification of Death Facts”, which will include the name and date of the event only.
