ImmTrac is the State of Texas Immunization Registry. An immunization registry is a secure, confidential database that stores your child’s immunization information electronically. Authorized professionals such as doctors and school nurses can access and update your child’s shot record anytime at no cost.
Purpose: ImmTrac was implemented for the best interests of Texas Children. It is a free service that is designed to help children receive the right shots at the right times. This service plays an important role in protecting your child from many dangerous diseases. It also helps avoid unnecessary shots because it would contain a record of all the child’s shots. If your child changes doctors or schools in Texas, ImmTrac eliminates the hassle of transferring records.
Through ImmTrac, we will be able to safely and securely maintain your children’s shot records for you. We want for your child to always have accurate and up to date shot records and we firmly believe that a registry where your doctor can be reminded of upcoming shots or be shown what your child has already been immunized for, will greatly aid in your child’s health and well being.
HOW DOES ImmTrac WORK? When a health-care provider gives your child an immunization, and you consent to registering this information, the statewide immunization registry, known as ImmTrac, is notified. ImmTrac will keep an electronic immunization record on your child. Some information contained in the registry is the child’s name, date of birth, address, the name of the parent or guardian, information on the shots given, and who gave them. The child’s social security number and mother’s maiden name are optional.
HOW WILL ImmTrac PROTECT MY CHILD’S PRIVACY? ImmTrac is a confidential service and was created with only the best interests of Texas children in mind. As a result, your child’s records are available only to persons authorized by law to see it. Only doctors, schools, childcare centers, and public health providers with ImmTrac-issued identification numbers and passwords may view the information. |

HOW DOES ImmTrac HELP MY CHILD? Providers authorized to use ImmTrac can see what immunizations your child has already had, even if they were given in another city or county. So when you come in for shots, your child gets only those that he or she needs. This means that your child won’t be under-vaccinated and more susceptible to diseases. Your child won’t be over-vaccinated either, so he or she does not have to go through any more discomfort than is necessary. ImmTrac can also print out a shot record you can use to get your child into school or daycare.

HOW CAN I REGISTER MY CHILD? You can register your child in a variety of ways:
The most convenient registration is to come to Comal County’s Public Health Department and request that your child’s immunizations be entered into ImmTrac. You will need to bring a copy of his or her immunization record if you have not had immunizations done here before.
Or, when you take your child to a clinic or to the doctor’s office for shots, ask if they use ImmTrac or send their shot records to ImmTrac. If they do, give them your consent to have your child’s records put into ImmTrac.
To register your child for ImmTrac participation directly through the ImmTrac Registry, you must grant consent in writing by:

Completing, signing, and mailing or faxing to ImmTrac this
Immunization Consent Form, along with copies of any
immunization records you have for your child. You may mail

v Completing and signing the ImmTrac Registration Form offered by the birth registrar during the birth certificate process (newborn children only).
v Calling the Immunization Branch at (800) 252-9152 to request an ImmTrac Consent Form. Consent is required to be obtained only one time, and is valid until the child becomes 18 years of age, unless the consent is withdrawn in writing.
